Personal Spiritual Health Evaluation
The growth of a disciple follows the intentional process of development from the child phase to the parent phase. Each phase is unique in its focus, but the overall goal is Christlikeness in both character and competence. Each phase is described here as a “play.” There are five plays in the discipleship process. Each play is an important stage in growing as a disciple of Christ. The final stage is the reproduction of a disciple in the life of another.
Print this form and fill out. Evaluate yourself on a scale of 1-10, 1 being totally out of agreement with the statement and 10 being totally in agreement with the statement. After the evaluation you should state why you have given yourself that rating.

Assessment and goals
2020
Review each of the 5 plays and your answers of personal assessment. In each area, try to find one growth step you can take over the next year that will help you progress in that area. For example, if you rate low in Bible reading/study, you could set as a goal to read the Bible once a week. Do this in each of the 5 plays, looking for practical ways you can grow in each area.
Accountability is good! Please consider getting into a “triad” or “quad” discipleship group with others who also desire to grow spiritually. Your small group would be a great source for this.
Keep this assessment in a place where you can refer to it often. You will be given the opportunity to complete an assessment at the beginning of each year. The yearly results should help you define your path of spiritual growth.
